Manually created orders with customizations are duplicated on invoice

Whenever I create a new order in the back office and add a product that has customizations, the product's price is duplicated on the invoice.

 

This started after I upgraded from 1.6.0.9 to 1.6.1.1 and i have not been able to find a solution.   Last night I upgraded to 1.6.1.2 and i am still having this problem.

 

Does anybody know of a solution?   I create quite a few manual orders a week and this has been a real problem since the upgrade.

Prices are correct and all orders process correctly when placed by a customer through the front end.    There is no error with the price of the product.

 

The problem occurs when I manually create an order in the back end.   If a product has a customization, that product's price is doubled as if they are charged once for the product, and then again for the customization.
