Insert a php form problem, and invalid carrier or no carrier selected.

Hello

I will need some help regarding two problems:
First I want to put a form into the place of contact field. You have attached the form archive (I also tried to modify contact-form from the parent directory and from the theme).
Second I want to remove the delivery price and delivery altogether from front office.
Now the main problem is that I get an error “invalid carrier or no carrier selected”. This error is displayed when I try to checkout from the cart at the service tab.
I do not need the delivery because I sell services.
