I did removed all carriers and then created my own and it seems to be working now. Let's start with the simple question: do you have modules that in any way interact with shipping? It can be shipment module, delivery dates module and so on... Is your shipping cost and locations configured properly (shipping->carriers)? Does the carrier for selected address (or price/weight) exist?
