Adding fields on order page

I need to add a field on the page where the order delivery info is filled out. Specifically, I need to be able to have customers type in a license number that will be applied to the order @ the same time the address and customer name is filled out so it would look like this,

Company__________
First name *__________
Last name *___________
Address *_______________
Address (2)__________
Postal code / Zip code *_________
City *__________
Country *____________
State *____________
***LICENSE#***____________
Additional information____________
Home phone____________
Mobile phone____________

I need this info in the cust info field for legal and accounting purposes. Any ideas?
