Blockspecial module how to show only products with "on sale" flag ?

Hi. I would need to change blockspecials module to show only the products with the "on sale" flag activated"?

I have no need to show all the products with special prices but only those with the attribute on sale.

I suggest that you create override/classes/Product.php and then copy the getRandomSpecial function from classes/Product.php and change all WHERE to WHERE p.`on_sale` = 1 AND to filter out specials that aren't on sale. If you want the same on the "Prices drop" page, you'll need to copy the getPricesDrop function and do the same.
