Add custom fields in the carrier in admin panel

Hello,

      

         I want to add two custom fields in the Back Office -> shippings -> carriers -> click on add new

 you can see the form, i want to add email and phone number in the carrier option just after Carrier name text box.Please help me to encounter this problem.
