permanentlink do not root to the right page

Hi,

 

I try to set the permanent link on the top manually because no configure menu.

 

I went to the blockpermanentlinks-header.tpl

My page is normally contact-form.php URL contact and sitemap.php with url site map.

 

SEO url friendly is activate. 

 

Any help would be appreciate here also I still can not see my image on the front page . (home default has been created in the images section.
