I had this problem with PrestaShop 1.5.2 a didn't solve it even with 10 attempts. I just waited to v1.5.3 and the problem is gone.

try switching your db type from InnoDB to MyISAM...when asked for type

iv change tried several db names and user name because my provider said that was the problem than tried change db type and i cant get out of error on populate db, im sure this is a server problem but my hosting dont have any idea.
