Form for applying resellers?

Hi,

There is a big possibillity that this topic has been adressed before but I can't seem to find it. So sorry for double posting if that is the case...

What I am looking for is a way for applying resellers to fill in a form with a few, from me, choosen questions about them. If they have a store and some other info that is good to know before they get approved of the reseller discount we have.

So just about 10 questions to fill in and then press a send button and the form goes to our e-mail would be ideal :).

Is there such a module available (for latest version of Prestashop) or is there another way to integrate such a form on the webshop? I.e a java script or something?

Hi, addon's has several form builder modules. native PS does not support.  You could put in a google form, jot or other type integration...

The one I used is the free formmaker from Hubspot (https://www.hubspot.com).

Works very well and is simple to set up.
