Extra checkbox option on delivery method

I would like to add a checkbox at my delivery options so my clients can choose to "do/don't give the order to the neighbours when I'm not at home". See screenshot. 

 

The option doesn't has any effect on the price, it's just some extra information. 

It has to be shown in the order's information so I can check it quickly. 

 

I know I can create a new delivery method for it, but I'm planning to have multiple delivery methods, and the "neighbour delivery"-option would mean I have to double all the delivery methods. Which is not the best solution. 

 

Didn't find any module who can arrange this yet. 

Any thoughts?
