Google Analytics issue - Track only index.php page

Hello, I have issue with Google Analytics v2.3.4.
 
Module, probably track only index.php (homepage) site.
 
When I check "Content overview" at my GA account - it show only index.php - no other sites like product pages, CMS, etc. (see attachment)

 

Note:
1. I set up API Google Analytics v1.2.2 too
2. I tried install / reinstall both modules

 

Script including at all pages:
