CMS PAGE PHP

Hello,

 

I have created a new page CMS. Html and CSS it's ok.

 

But I dont know connect the fields to the database, I need help!

 

 

I put the HTML in CMS.tpl inside  {if $request_uri|strstr:"16"} - that's ok, it works! 

 

 

Now, I have that fields:

- Name 

- Contact

- Email

- Date of Birth

 

Now how I can connect this fields to database? I need help to develop the code and where I put the code?
