Cant switch themes.

I switched to the default theme to troubleshoot some issues and now I cant switch back to my normal theme. I get:

 

 Duplicate entry '81-0-1' for key 'PRIMARY'

 

INSERT INTO `mpj_hook_module` (`id_module`, `id_shop`, `id_hook`, `position`)

                  VALUES (81, 1, 0, 1)

Also note I tried to install a completely different theme and I still get the same error when I try to switch to the new theme!

Newest update: 9-29 7pm MST

 

Got my host to restore a backup from last night (files and the PS database) now I have a blank website. I can access the backoffice but the front office is just a blank page.

its in dev mode and the closed for maintence page returs this error:

I tried switching themes but it was still a blank screen. It appears to be a "addJsDef" related problem.....Im exhausted for now, will continue tomorrow morning or its my head!
