Edit page title/h1 via layered navigation block

Hello everyone,

 

Here is the problem: 

When I apply some filters using layered navigation, their values are appended to page h1 header, which already contains current category name, so I want to append the filter value to h1 only if there is one filter applied, otherwise, I want the h1 header to contain only category name and make page's title to be the name of current category
