Contact Us Form Not Sending Information Correctly - Need this fixed please.

Hi everyone! I have a job! (Probably pretty simple.)

This is exactly how I want it. However, when I send myself a test email using this form, I do not receive all information.

 

Customer e-mail address: (I get this)

Full name: (I do not get this)

Customer message: (I get this)

Order ID: (I do not get this)
 

Attached file: 3.jpg (That's all I get, a text simply stating "3.jpg" but I cannot click on it to open the image file, it's simply only text.)

 

You'll need to go into my contact.tpl file or whatever it is and fix this, I think.

Thank you so much for the tutorial! But I'm simply a business man, not a website coder/programmer.

 

And even if I added that name field correctly by myself, I'm still not receiving the order reference number when someone's not logged in and enters it manually. Also the image file does not send as a clickable/viewable attachment, simply a text stating what the image/file name is on their computer.
