SSL padlock icon not displaying in firefox browser

Hi,

 

After installing ssl certificate and enabling https from backoffice padlock icon not displaying in firefox browser, IE and Crome is ok.

 

 

Below error when testing on "whynopadlock.com"

 

Insecure <form> call.
Found on line # 1 in file: www.fastyl.com/login?back=my-account

 

 

Using 1.6.0.8v with default theme.

 

 

Help would be highly appreciated.
