issue with checkout process using COD and cheque or even bank wire

Hi friends
Pls help me i'm using PS 1.2.5
i am facing a problem where when i select checkout by
Cash on Delivery = i get //modules/cashondelivery/validation.php on url

but what happens is the complete format gets haywire of the page

a complete list of category list down on the page and which pushes down the confirm button down under the page see here in the attachment.

Pls help as my site is already live and customers facing this isue

its some issue with the wiznav ..categories.
when i unclick the catergoris tab in wiznav ..it goes well.

i think for now its resolved .i just unclicked the categries from wiznav
