Tweaks / Addons for a flower like shop in PrestaShop 1.6

Hello,

I am developing a flower shop e-commerce website with prestashop 1.6 that sell international and i want the user be able to choose destination and desired delivery date.

I am thinking something like this: a box that includes 3 or more dropdown fields with "Occasion" , "Destination (one dropwdown with countries and another with municipalitites if needed (for example for Greece i want to have also municipality) and "Desired Delivery Date".

Can you help me on this? I search but the only addon i found is this one http://www.ecommy.com/product/prestashop-delivery-date for delivery date option in the product page.
