Job offer - report module (customer data)

Hi,

I'm looing for somebody, who can create a module with following functionality:

 

From a list of all clients I can choose one customer for which will be created the report (on one page) with following data:

1. Information - customer data

Name od field                Values from column: (table address)

Customer ID                id_customer
Company:                    company
Lastname                     lastname
Firstname                      firstname
Adress                          address1
                                     address2
Post code:                    postcode
City:                              city
Other info:                    other

Here I need all addresses of customer with the same ID
