Simple contact form modification

How do we set only one option for the subject heading ie Customer service in PS 1.5.2 default theme?

 

Also I want to remove the attach file option for the user.

Ok solved first part. Simply delete contacts in BO.

I guess you can comment out or delete lines 117-123 in file: themes/default/contact-form.tpl.
