Shipping fees are not working zone wise basis

I have build a custom carrier for my shop and include 3 zone like USA,CANADA,OTHERS for different shipping fees, also I changed all countries zone into my 3 new zone. But when continuing my shipping only USA zone's fee is shown for all country. I have added screenshots of my problem please help me.

Have you followed this below exactly in Back Office > Shipping > Carriers sub tab

 

How do I create a new carrier?

• Click "Add new".
  
• Fill in the fields and click "Save".
  
• You need to decide on a price range or a weight range for which the new carrier will be available. Under the "Shipping" tab, click either "Price Ranges" or "Weight Ranges".
  
• Click "Add new".
  
• Select the name of the carrier and define the price range or the weight range. For example the carrier can be made available for a weight range between 0 and 5lbs, and another carrier can have a range between 5 and 10lbs.
  
• When you are done, click "Save".
  
• Click on the "Shipping" tab.
  
• You need to set the fees that will be applied for this carrier. At the bottom on the page, in the "Fees" section, select the name of the carrier.
  
• For each zone, enter a price and click "Save".
  
• You are all set! The new carrier will be displayed to your customers.
  

I have done this twice this morning following the above exactly and it works for me, I am using 1.4.7 and 1.4.8

My best advice would be to make a separate carrier for each zone with the same name and different costs.
