Need help to put origin and destination in cart block

I am using Prestashop for my studies project. My project was to create applications for the moving company. Moving company is a company which is engaged in the transfer of clients goods . Suppose we want to move house, then we hire "Moving Company".

If you want to try, go into the service menu, then to household items. Then try to add one-by-one the goods. The application will transferred all the goods calculation price which will appear in the cart block to the up-right.

Well, I want put script in the block cart so that it can select origin and destination and date. So it will looks like a airplane ticket online web. The purpose is to let the user to select his origin house and destination address place in specific date. Please suggest how to enter the script ?
