Blank page at checkout with bank transfer - no account details or mail

After customers check out with bank transfer, the page that is supposed to show the account details is blank. I have entered all details in the module. This only seems to happen to some customers including myself when i try out my site, because a lot of my customers can still pay with bank transfer. When the page is blank, the confirmation email is not sent out either. Anyone knows what this is?

This seems to be quite a common issue. I've been searching for a solution the whole day now without success. The weird thing in my case is, it only happens once in every 30 orders or so...

I turned on error reporting and now I'm just creating orders to get this error again so I can see where it goes wrong.

This could be related to a recent Swift (Mail) error that I started seeing around.

I did a search about it (as I noticed it on a couple of sites we manage without anything changing), and found a post about it on the French forum http://www.prestashop.com/forums/topic/178615-swift-message-mimeexception-probleme-de-commande-fantome-et-mot-de-passe-oublie/
