I can't check the age conditions to create an account on my e-commerce web site

Hi everyone, thanks in advance if you can help me,

sorry for my bad english, i'm a french developper.

 

I've a checkbox to able users to select that they've the good age to access to our wine e-commerce web site.

But the <input> who able the check is retarded compared to the small square display at the right of the text (see screenshot bottom).

 

I didn't arrive to move the input on the square. I used  use HTML Block but I really don't understand the plugin utilization.
