How to add form to custom module

Hi, I am working on a module to make specific product configuration possible. Cant you point me to some resources, where I read about adding form to my module? On form submission I want to insert some records to database, so user can later pick those values and assign them to specific products. 

Some example code, repo, link to docs - anything.
