Blank site after upgrade

I just upgraded from 1.3.7 to 1.4.1 by following guidelines.
All appeared to go well and I can access new Admin area, but front-end is blank.
I enabled debug modes in config.inc.php and received:

Warning: Missing argument 2 for ToolsCore::getMetaTags(), called in /home/username/public_html/init.php on line 108 and defined in /home/username/public_html/classes/Tools.php on line 574

Fatal error: Call to a member function getCategoryLink() on a non-object in /home/username/public_html/modules/blockcategories/blockcategories.php on line 129

I need help understanding what this means and how to go about fixing it to get my site back.

UPDATE:
I dropped the database and uploaded all of the 1.4.1.0 files again. Ran the install script again and this time - it worked - sort of...

See the result? I have no clue what is causing this. I tried specifying Smarty3 in the Admin area - resulting in crashing the entire site.
