Add required field to Freight/check-out or pick-up module

I need to make one or two fields required when checking out. I need some data back from my customer. I´m going to use it to get information about the customer. When we offer a pick-up service we want to know which training-group they are in and the name of the person doing the pick-up. Not allways the same name as the person doinung the order... This to be able to know when and where the order should be left for pickup.

 

Anyone got a tip of the "right" module?
