Hcatmenu.php file from a third party template causing an error on the page when installed

Hey Everyone,

I have uploaded a template from template monster and am having an issue with Horizontal Category Menu v1.0 Module.

There seems to be an issue with the hcatmenu.php file. Everytime i add this file to the module directory hcatmenu, my website comes up with a 500 error.

I have attached the code below, perhaps someone can help me get this module active on my website...
