How to add multilanguage input

Hello 
I have problem with add multilanguage input text form. I would like create form with three input i.e. title, description and url. All input must be multilanguage, how can i do this? Next step is show title and description on front controller in the right language.

I know how show anything on front controller, and how can i show anything on back office. But i don't know how i can add form input with multilanguage input, and how show this value of front in the right language.
