Friendly URL's not working in custom theme

My custom theme works except for all of the urls in the Preferences > SEO & URL's panel. When I activate the default bootstrap theme these friendly URL's work fine but when I activate my custom theme they don't work and I get the error 500 page. I have activated and de-activated Friendly URL's to regenerate my .htaccess file but still have the same problem. I have also checked with Bluehost that mod_rewrite are enabled on the server level and they are. I don't know what the problem could be? Any help would be greatly appreciated. I have tested the same setup with my current theme locally and friendly URL's work fine.

when there is a 500 or blank page, then we must have the underlying error for resolution, turn on ps debug mode and try and get the error (also you can check your hosting php error log)
