Losing Address information

i use express order in my prestashop, but when i use "create account" it was get some errors and redirect to order express. look my picture in my attachment or try to create account in my shop www.d1500.com. how can i fix it??

now i try it with another account and browser (before it i use firefox). and it was no problem. look at my attachment. why it was happen? before that problem happen, i edited user's email in database manually and i tried to create new account with email before. here illustration what i have done :

step 1 : i create account with "[email protected]"
step 2: i rename email manually in database, so it was be "[email protected]"
step 3 : i create account with "[email protected]" again
step 4 : error like my post above

any sugestion or explantation ??
