Fedex Module not showing ground delivery

I've found to many threads with the same problem but no real fixes.

 

My Fedex module won't show ground delivery. Instead it shows 2 day shipping, fedex express shipping, standard shipping but it won't show ground delivery. Even if no weight or dimenisons are set it still brings up only those shipping options. And if I disable those shipping options under carriers and leave ground delivery alone it will just say no available shipping options at the order page.

 

Also it doesn't matter where I add handling fees either under the product or under the fedex module it doesn't increase or decrease the shipping prices available at the order page. 

 

Has anybody actually ever gotten the fedex module to work?
