Aggiungere campi di testo in form contatti

Ciao ragazzi,

ho aggiunto dei campi di testo nel file contact-form.tpl... Il problema ora è che cosa devo aggiungere in contact-form.php? E soprattutto in quale? Perchè il mio contact-form.php è il seguente:
