Problem with Datepicker with hours

Hi,

I know is a long shot asking for help here.......but I Recently I bought the Datepicker with hours by ecommy.com 

but I'm having problems with it: and still waiting for the developer's help (few day by now)

after the installation just doesn't work meaning....

the shipping screen where the date picker should work just doesn't show up... it shows "server error" message..

I'm tried on a new prestashop installation 1.5.3.1




I installed Datepicker by ftp upload of the directory Datepicker to modules, and also tried uploading the zip but it just not working

Can someone help me?
