How to add 'Once-Off' fixed price based on product?

Hi, does anyone know how add a fixed charge based on a product?

 

I am building a shopping solution for a t-shirt manufactory, which require a 'once-off' charge for each new design (apply to the t-shirt). This 'once-off' charge will appear at the check-out. 

 

i've tried using 'CART RULES', 'CATALOG PRICE RULES', that did not work. 

 

i've also tried modifying the rules under Product tab 'COMBINATIONS', and 'PRICES'. But, that still didn't work.

 

So, any idea??
