Choice per-item shipping or in-store pickup

I need a product to have a choice in shipping selection:

Option 1: in-store pickup (no charge)

Option 2: ship to customer (with a $5.00 per item charge)

 

So far, I can:

- get a $5.00 per item shipping added in the cart

- get a choice of carrier options (in-store or mail-to)

 

But, I can't get what I need:

- choice of shipping/carrier options BEFORE the per-item shipping gets calculated

 

Any idea if this is possible? If so, how can I implement it?
