How I can customize a product?

I'm riding a sporting goods store that contains as club shirts, boots, etc.. Prestashop version is 1.4.7.0.

 

I would like to customize the product to the customer when you add a shirt to cart can specify for example in the case of buying a shirt the name and number on the back, or in the case of a boot the number, type of embroidery , etc. If the customer selects customize the product price increase will occur as you select. With customization options I've seen of prestashop, editing product allows me to add any text or image fields but not quote a price associated with the field, nor preloaded data to be a closed list or indicate whether it will be a check , a list of options.

 

Is there a module that allows this advanced product customization? Ideally, you could configure everything from the back-end
