rewriting (mod_rewrite) is not working for new page(controller)

Hello Everybody

 

In Prestashop 1.4, I had made a page with a controller and it was working properly in Friendly-URL mode.

But when I did the same in Prestashop 1.5.3, it gives me page-not found error.

 

The page is working fine if the Friendly-URL is disabled.

 

I checked everything but failed.
