Error: This country requires a state selection.

I am using 1.4.2.0 and customers have a problem adding new USA addresses.

The New Address page does not give the option to select a US state. The select state drop down box appears when creating an account but does not appear when adding a new address to an existing account.

I have read through the many other posts with a similar problem but they were for earlier versions of PS and none of the solutions work for me. What is different with my issue is that the State selection does appear when creating the account by disappears when adding addresses.

My theme was the default theme from 1.4.1 and I have removed the age, gender and date of birth code.

Any help would be much appreciated.
