PayUnity.Flex Credit Card manual integration

Hello I am trying to manually integrate PayUnity's credit card payment option.

At point 2 I have a question:

In which file do you enter these two lines? (<script src ... / <form action)

I am using prestashop 1.6.1.17

I can't use the modul from PayUnity because I am using not the standard Prestashop checkout.

 

I hope someone can help me.
