Custom fields on registration PS 1.7

Hi everyone, I have to insert mandatory and non-mandatory fields in the registration form. Use of PS 1.7 (no B2B) below, I list the current status of the items in the registration field:

The bold terms are those to be added to the existing ones and I should also make the date of birth mandatory.

- First name
- Surname
- Date of birth
- Fiscal Code
- City of residence
- Email
- Tel
- Password
- New password

Also I should add a radio button with the possibility of entering text in case the customer selects Yes rather than No.

Can you help me please? I'm going crazy, I can't even find modules that are right for me.
