More than 1 product combination is not added

Hello, 

 

I am trying to add product combinations but more than 1 combination is not getting added. I tried using both the product combinations generator and adding manually. The screen refreshes and says "Updated successfully" but only 1 combination is saved. I don't even have too many combinations -- max of 5-10 combinations per product. I tried checking by DB and all seems fine except for the product_attribute_combination table (attached screenshot). Not sure if this is how it should look or if it is an issue. 

 

Please help!!! I cannot continue using my site without this...

I am still having this issue. Can someone please help. I was able to figure out that the sql query for inserting records into the stock_available and product_attribute_shop are not getting fired. But am not sure how to fix this or if I should be replacing any controller files. Any guidance is appreciated as this is only issue stopping my site from going live.
