Creating a returns form on 1.3.6 that will will be emailed to shop address on completion, any one do this already?

Hi,

I am thinking of creating a returns form for an electronics store. I would like a client to click on 'the link' withing the returns page and a form page should be displayed, on completion it should be emailed to the shop's email address.

Any suggestions on existing modules that I could edit to achieve this or is there something like this out?
