Shipping fees not getting added in Cart

This has happened a few times over the last few months. There doesn't seem to be any pattern to it. I have free shipping on all orders over $150.00 but there are times when an order for less than $150 gets free shipping. I do not have any coupon codes active. Can someone please help me figure out what is wrong? It's very frustrating to contact a customer and ask for more money or refund their order. I'm forced really to honour the price paid.

That’s the strange thing. Most of the orders it works fine. But once in a while - I would say about 4 times in the last 6 months it doesn’t. Products on each order are different so it’s not product specific. Happened again yesterday which prompted my inquiry.

Can you please let me know if there is any specific scenario?

Because we need to reproduce the scenario to give any solution, without reproducing there will be no solution.
