And what if user is adding two different products to cart. One with warranty and the second without, or user want to select different warranty times for two dif. products ? So you cannot realize this by cart option. The best way to realize this is only directly with product option.

If you add it as an attribute like I told before, than it would be shown on cart and also on invoice. Simply try it out. If you are using own theme, so perhaps this function is missing. On Prestashop default theme the attributes are showing on check-out process and also on invoice.cart.thumb.png.cb2edb9e21f1e7fd14bbbe45c6e899da.pnginvoice.thumb.png.9ecbd4b79aa4051881f724c4f9be42f5.png

Prestashop only shows prices as whole. If you want the impact price showing as extra, than this is a customized task. If you are not a developer, than you should contract somebody for to make this change on your theme in use. You will not find any module for that.

Take PS 1.6. cause it is stable and is having a fix architecture. You should take a look into the file /themes/yourtheme/product.tpl. There you will see how prices are formed. Than you need to change it also on other files which are leading with prices.

PS 17. is a version in development, what means part smarty architecture, part symfony architecture, with things changing on each new version update. File you will find at /themes/classic/templates/_partials and /themes/classic/templates/catalog

I have a module named Order Insurance, which might help you here.

It allows the customer to add/remove "insurance" to the order, based on a predefined rate that you can define in the modules configuration page.  Perhaps this solves your need, or at least gives you a starting point
