Add custom field in newsletter block for targeted emails

Hi I am trying to add a dropdown in newsletter block to allow subscribers to select interests from a dropdown fo future email newsletters.

I've got the dropdown part on the blocknewsletter.tpl working, and the 'interests' displaying in module admin, but data not being written into database.

I have a similar dropdown on OPC page that works and is collecting interests data fine, just not working on the subscribe box

Have attached files - anyone help point out what I'm missing?
