Extra dropdown fields

I'm searching a module which allow me to add 4 dropdown menu's on a product page, where visitors can choose some extra settings for the product. (see screenshot)
The values of the dropdown choices must be printed in the order mail, invoice pdf and in the order in the backoffice. 
Is there a module for 1.7.2 that can do this? 

(the idea behind is that I have a giftproduct where customers can choose 4 products from a 20 productpool. I tried it with combinations, but it created too much combinations to handle it efficient)

When you add an attribute you can choose the way to be shown, so change the attribute Kreuzeproduct 3 to dropdown list:

All attributes are printed on cart and invoice. This is native Prestashop feature.

screen is PS 1.6, but same on PS 1.7. Tab: Catalog -> Attributes & Features -> add new feature or change one there.
