Form generator

Hello everyone ,

 

I am looking for the best form generator according to the community's experience. Here are my criteria :

 

I want it to be simple (can add logo and fields easily), can perform verification for email address for example

 

Can link the form to any button of my application easily

 

I want to use it to allow a user to request for a quotation clicking on a button which means a text "reference" would be auto filled when arriving to the form

 

I also want to send an email of confirmation to the user that the form information has been received on my side.

 

If there are templates for usual applications (such as request for a quote), that is even better
