Simple Form Field Validation

Im going to create a function to handle returns outside Prestashops own return system. For this I would need a page that asks the customer for his order reference, and when a customer enters it I want the page to check if the reference number matches an order in my database/shop. If its okey I want the customer redirected to a cms page where I want a customized contact form that would be sent to my e-mail adress.

 

I would prefer if it was possible to handle and give me the code/instructions (im quite well aware of the prestashop structure etc) since Im not able to give full access to bo or fo.

Im using Prestashop 1.6.1.1.
