1.6 Customer doesn't get any order confirmation after completing order

After finishing the last step (5) of making order there's no: confirmation like

'your order is ok' + redirection to home + email.

 

Instead the user just stays on step 5 and the cart gets empty (the order was received)

 

If the user clicks again confirm there's an error 'Cart cannot be loaded or an order has already been placed using this cart' but that's ok (because the order was already made).

From the backoffice the order is received correctly.

 

Did anyone have similar issue?
